Diabetes for Primary Care in 2012

Program Overview

There are nearly 26 million people diagnosed with diabetes and another 57 million are at risk for developing diabetes during their lifetime. With the growing epidemic of this chronic, progressive disease, the need to improve diabetes care is paramount. The vast majority of adult patients with diabetes are cared for by primary care clinicians who face significant challenges, along with diabetes educators and other professionals in managing this complex disease.

This one-day symposium addresses these challenges through presentations and case reviews developed by recommendations from previous attendees and the St. Luke's Diabetes Collaborative Practice multidisciplinary team. Presentations are designed to promote interaction between the faculty and the participants to promote change in the care and management of the person with diabetes.

Target Audience

Primary Care Practitioners (Physicians, Nurse Practioners, PA's), Diabetes Educators, Nurses, Dieticians, Medicine and Family Medicine Housestaff.

Learning Objectives

At the conclusion of this conference, the participant should be able to:  

  • Discuss lifestyle interventions for nutrition in the continuum of care for Diabetes
  • Identify factors that lead to provider burn-out in managing patients with Diabetes and strategies for coping with them
  • Describe how electronic medical record (EMR) can be used to improve the quality of care for people with Diabetes
  • Describe how to optimize Diabetes, Lipid and Cardiovascular medications in the management of patients with Diabetes and Cardiovascular Disease.

Texas Heart Institute at St. Luke's Episcopal Hospital designates this live activity for a maximum of 7 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

Nursing: 7.0 contact hours will be awarded for successful completion of this activity. To receive a certificate of completion with awarded contact hours, you are required to sign in at registration, attend the entire program, and return the completed evaluation form. 

St. Luke's Episcopal Hospital Department of Nursing and Patient Education is an approved provider of continuing nursing education by the Texas Nurses Association, an accredited approver by the American Nurses Credentialing Center's Commission on Accreditation.

Dieticians: Approval for 7.0 CEU for Registered Dieticians has been applied for with the Commission on Dietetic Registration(CDR).

An evaluation form sent post event will provide each participant the opportunity to review the speakers, to identify educational needs, and to comment on any perceived commercial or promotional bias in the presentation.

Cancellation Policy
All cancellations must be received in writing by Friday, August 31, 2012 in order for a refund to be issued. A $25 processing fee will be assessed each cancellation. Refunds cannot be made for those who register but do not attend the scheduled activities. Texas Heart Institute reserves the right to cancel this activity if registration is deemed insufficient.
